| Care Management Department
All
of the services under Care Management are designed to help HIV-positive
registered GMHC clients meet their basic social needs and help them
cope with living with HIV / AIDS. We provide stability in an increasingly
difficult environment by addressing a range of quality-of-life issues:
food, housing, access to health care and mental health services.
We support individuals working to improve and transform their lives.

Intensive Case Management
The Intensive Case Management Program is located on the 4th floor.
If you have Medicaid or are Medicaid eligible and live within New
York City's five boroughs, and need help in accessing medical and
social services, you and your family can receive on-going help from
our trained staff. Our staff will work cooperatively with you to
provide support, advocacy, and assistance with entitlements, housing,
medical care, domestic and family problems, budgeting, substance
use, and mental health issues as well as access to education, work
and volunteer opportunities. Walk-in hours are available daily.
Learning Center
You may want to enroll in the Learning Center on the 12th floor
and take advantage of a variety of services. The Theater Desk distributes
tickets to entertainment, cultural and sporting events. Books for
your reading pleasure are available at the Lending Library. Our
Continuing Education Program provides scholarships to local colleges
and universities. Return-to-work support services are provided through
our partnerships with various vocational agencies. Additionally,
you can participate in our social events and classes and even sign
up for haircuts. The Learning Center is constantly evolving to better
help you redesign your life. You may access the Learning Center
by requesting it when you go through Access and Assessment, and
then attending one of our two monthly orientations.
Life Steps
Is Life Steps right for you? Is your housing out of control? Can't
deal with the house rules? Afraid you will lose your housing? And
on top of everything, are you all alone? You may be helped by our
Life Steps program. Need support? We have groups, and Life Steps
can help if you are missing out on medications and doctors.
Life Steps is all about support. The program has groups, one-on-one
work and activities. You get connected with people, use less alcohol,
use fewer drugs, feel better and get services to prepare you for
permanent and stable housing. We use the Internet and computers
to learn more, and to get ready for an education and a job. Nutritional
snacks and Metrocards are given for attending the program. Life
Steps is located on the 12th floor.
Meals
If you want to enroll in the 12th floor Meals Program, you can
receive a hot nutritious meal in a setting that permits healthy
socialization and fosters exchange of helpful information. Lunches
are served Mondays through Thursdays and dinner is served on Fridays.
Special meals are served on holidays. Through its tabling program,
you will also have access to information from other GMHC programs
and outside agencies, such as the Bailey House Conflict and Mediation
Center. You may access the Meals Program by requesting it when you
go through Access and Assessment
and then by attending one of our two monthly orientations.
